During a recent sprint, a bug made it all the way to production. From a
QA standpoint, what would you do to ensure that the same issue isn’t

Situation:
In one of our recent sprints, a defect related to pricing calculation went live in production, despite having passed all our test cases and automation checks.
Task:
As the QA lead, it was my responsibility to investigate how the issue escaped detection and implement measures to prevent recurrence in future releases.
Action:
I immediately conducted a root cause analysis involving the QA, Dev, and Product teams to trace how the bug slipped through. We discovered that the scenario wasn’t covered in our test cases due to a gap in regression coverage and missing edge-case data in our test environment.
To resolve this:
● I added new test cases that captured the missed scenario and updated our test case repository.
● I wrote a new automated test script replicating the real-world customer journey that triggered the issue.
● Integrated this script into our CI/CD pipeline so it would run on every build.
● I also audited related features to identify similar blind spots and proposed enhancements to our QA checklist during sprint reviews.
Result:
These changes significantly improved our test coverage. We prevented similar bugs in later sprints, and our QA process was seen as more proactive. The issue also led to improved cross-team alignment on edge-case scenarios during backlog grooming.
